Forms help you collect information, feedback, or responses from your audience in an organised way. On CircleHQ, you can create two main types of forms: Survey Forms and Embeddable Forms.

How to Get Started

1. From your dashboard, go to Forms.
2. Enter a title and a short description of your form.
3. Choose the type of form you want to create:

1. Survey Forms

Survey forms are great when you need to collect responses directly from users. When you click Survey Forms, you’ll see options:

• Regular Forms – simple forms for everyday feedback and data collection.
• Legal Forms – for documents or agreements that require compliance.
• Custom Forms – design your own to fit your needs.

Once your survey form is ready, click Publish. You’ll then receive a shareable link to send to your audience, or a QR code that you can download and have them scan.

2. Embeddable Forms

Embeddable forms enable you to place a form directly within your website, documentation, or portal, allowing users to complete it without leaving the page.

Steps to Create:

1. Choose a template (or start from blank).
2. Edit the elements and container to your preference.
3. Click Continue.
4. After submitting, you’ll see three options:

→ Popup Embed

• Choose the position (centre, top left, bottom right, etc.).
• Set a time delay (in seconds).
• Generate the embed code.

→ Form Link

• Copy the link to share directly.
• You can use this in emails, chats, or social media.
• Click Done.

→ iFrame Embed

• Copy the iframe code.
• Paste it into your website’s HTML to display the form directly on a page.
• Click Done.

Once you’re finished, your embeddable form is ready to use!
